The Case Manager will be responsible for the overall coordination and management of consumer care and services across the Ballarat and Gellong & Bellarine Peninsula regions. They will work closely with consumers, their families, and a multidisciplinary team of healthcare professionals to ensure the delivery of high-quality, compassionate care.
